Brain imaging of 49 children aged 9-10 has found that those who were physically fit had a hippocampus significantly bigger (around 12%) than those who were not fit. Animal studies and those with older adults have shown that aerobic exercise increases the growth of new brain cells in the hippocampus. Physical fitness was measured by how efficiently the children used oxygen while running on a treadmill. Fitter children also did better on tests of relational (but not item) memory, and this association was directly mediated by hippocampal volume.

Keeping active in middle age tied to lower dementia risk
A very long-running study, in which 800 Swedish women (aged 38-54) were followed for 44 years, found that women with a high level of mental activities in midlife were 46% less likely to develop Alzheimer's disease and 34% less likely to develop dementia overall, compared with women with the low level of mental activities. Women who were physically active were 52% less likely to develop dementia with cerebrovascular disease and 56% less likely to develop mixed dementia, compared with women who were inactive.

Exercise activates brain networks in older adults
A study involving healthy older adults (55-85) found that recall was better after a session of moderately intense exercise, and several crucial brain regions showed greater activation.
The recall task involved identifying famous names and non famous ones. The test occurred 30 minutes after the exercise session (using an exercise bike) and on a separate day after a period of rest.
Higher aerobic fitness levels linked to fewer word failures in older adults
A small UK study involving 28 healthy older adults (20 women with average age 70; 8 men with average age 67), has found that those with higher levels of aerobic fitness experienced fewer language failures such as 'tip-of-the-tongue' states.
The association between the frequency of tip-of-the-tongue occurrences (TOTs) and aerobic fitness levels existed even when age and vocabulary size was accounted for. Education level didn't affect TOTs, but only a few of the participants hadn't gone to university, so the study wasn't really in a position to test this out.
Exercise & healthy diet associated with better cognition
A Canadian study involving 45,522 adults (30+) found that higher levels of physical activity, eating more fruits and vegetables, and having a BMI in the normal weight or overweight range were each associated with better cognitive function in both younger and older adults.
Specifically, for those who were normal weight or overweight (but not obese), eating more than 10 servings of fruit and vegetable daily was linked to better cognitive functioning. When moderate exercise was added, those eating less than five servings, reported better cognitive functioning.
Stopping your exercise program can rapidly decrease brain blood flow
A small study involving 12 very fit older adults (aged 50-80; average age 61) found that, after stopping their exercise routines for 10 days, there was a significant decrease in blood flow to several brain regions, including the hippocampus and regions involved in the default mode network (both the hippocampus and the DMN show deterioration early in Alzheimer’s).
Obesity linked to inflammation & impaired brain function
A mouse study has found that obese mice had high levels of interleukin 1 in both their blood and their brains, and this was associated with:
- high levels of inflammation,
- low levels of a biochemical important to synapse function, and
- impaired cognitive function.
Moreover, when fat was removed from the obese mice, interleukin levels dropped dramatically, and cognitive performance improved.
Putting obese mice on an exercise program had a similar effect, even though they didn’t lose weight — but they gained muscle and lost fat.
Importance of muscle for older adults
Data from the American National Health and Nutrition Examination Survey (NHANES) III, involving 3,659 individuals (men aged 55+; women 65+), has found that the more muscle mass older adults have, the less likely they are to die prematurely. The findings add to the growing evidence that overall body composition — and not the widely used body mass index (BMI) — is a better predictor of mortality.

Physical activity saves hippocampus in people at risk of Alzheimer's
A study involving 97 healthy older adults (65-89) has found that those with the “Alzheimer’s gene” (APOe4) who didn’t engage in much physical activity showed a decrease in hippocampal volume (3%) over 18 months. Those with the gene who did exercise showed no change in the size of their hippocampus, nor did those without the gene, regardless of exercise. Physical activity was classified as low if the participant reported two or fewer days per week of low intensity activity, such as no activity, slow walking or light chores.
Exercise beneficial for dementia
A new review from The Cochrane Library, based on six trials involving 289 people, has concluded that exercise can improve cognition and the ability of older people with dementia to carry out daily activities, such as walking short distances or getting up from a chair. However, there was no clear effect of exercise on depression in older people with dementia, and the reviewers say that more evidence is needed to understand how exercise could reduce the burden on family caregivers and health systems.
