It’s been known that decreased blood flow in the brain occurs in people with Alzheimer's, and recent studies suggest that brain blood flow deficits are one of the earliest detectable symptoms of dementia. A study has now shown why it occurs: a small percentage of capillaries, the smallest blood vessels in the brain, are blocked by white blood cells stuck to the inside of the capillaries.

Recent research has shown that capilleries are vital for monitoring and directing blood flow around the brain.

Data from 3,105 older adults (65+) who had either heart surgery or cardiac catheterization has found that those who had heart surgery didn’t experience much greater cognitive decline compared with those who had the much less invasive, catheter-based procedure.

Two years after the surgery, surgery participants showed a greater amount of decline equal to only 4.6 months of cognitive aging compared with those undergoing catheterization.

A study involving 99 healthy older adults found that levels of monounsaturated fatty acids were associated with cognitive performance and the organization of the brain's attention network.

MUFAs are found in olive oils, nuts and avocados, and are a component of the Mediterranean diet, which has been associated with slower cognitive decline and lower dementia risk.

Data from AREDS2, involving 4,203 older adults with age-related macular degeneration, has found that daily dietary supplements of either omega-3 polyunsaturated fatty acids (also found in fish) or lutein and zeaxanthin (nutrients found in green leafy vegetables) were not associated with reduced risk for cardiovascular disease (CVD).

Creating amyloid-beta requires the convergence of a protein called amyloid precursor protein (APP) and an enzyme that cleaves APP into smaller toxic fragments (beta-secretase or BACE). Both APP and BACE are common in the brain, so why don’t we all get Alzheimer’s?

The jugular venous reflux (JVR) occurs when the pressure gradient reverses the direction of blood flow in the veins, causing blood to leak backwards into the brain. A small pilot study has found an association between JVR and white matter changes in the brains of patients with Alzheimer’s disease and those with mild cognitive impairment. This suggests that cerebral venous outflow impairment might play a role in the development of white matter changes in those with Alzheimer’s.

Two studies indicate that young people carrying the “Alzheimer’s gene” (ApoE4) do not have the pathological changes found later in life. The first study, involving 1412 adolescents, found no differences in hippocampal volume or asymmetry as a function of gene status. The second study, involving 173 young adults (average age, 28 ± 7.6 years), found no difference in plasma concentrations of amyloid-beta peptides.

Blood pressure data from 378 participants in the Framingham Heart Study has revealed that those who had high systolic blood pressure when they were 50-60 years old scored worse on a working memory test 30 years later. However, an association with verbal fluency was only significant for those with the 'Alzheimer's' APOe4 gene.

A study involving 30 children (aged 8-10), of whom 15 had experienced a sports-related concussion two years earlier, and all of whom were athletically active, found that those with a history of concussion performed worse on tests of working memory, attention and impulse control, compared to the controls. This impaired performance was also reflected in differences in brain activity. Additionally, those who were injured at a younger age had the largest cognitive deficits.

All of this points to a need for focused and perhaps prolonged interventions, especially for younger children.

Data from the Women's Health Initiative Memory Study, involving 6,467 postmenopausal women (65+) who reported some level of caffeine consumption, has found that those who consumed above average amounts of coffee had a lower risk of developing dementia.

Caffeine intake was estimated from a questionnaire. The median intake was 172 mg per day (an 8-ounce cup of brewed coffee contains 95mg of caffeine, 8-ounces of brewed black tea contains 47mg, so slightly less than 2 cups of coffee or less than 4 cups of tea). The women were cognitively assessed annually.
